Library management will review your application and inform you of your request status via phone or email.

NOTICE: Forms will not be submitted unless all boxes have been completed.

Also: Requests must be made at least 2 weeks out from desired date.

The library will work with the person listed above exclusively during the reservation period of this agreement. This person is the responsible party and must be present during the event to implement the Facility Usage Policy and Agreement.

I have read and agree to abide by the Facility Usage Agreement & Policy.

I understand and agree that a library representative must be allowed to enter the rented space at any time and for any reason, as stated in the Facility Usage Policy (General Rules #4).

I understand and agree that any recurring events can be altered for any reason and at any time as the Hendersonville Library sees fit.

 *

I understand and agree that repeated failure to check in, check out, or report attendance numbers may result in room reservation cancellation at any time.

 *

All room reservations will end 30 minutes prior to the close of business on date of reservation. Room must be reset to original layout, trash removed, and attendees have vacated the room.
